COMPENSATION: $12.00 – 15.00/hr

JOB TITLE: Case Manager RESERVE (Part Time  – no more than 20 hours/week) (Non-Exempt)

We are looking for RESERVE Case managers to enhance the quality of an individual’s life.  The successful candidate will be assisting the individual to achieve their specific goals.  Goals may include:

QUALIFICATIONS:

  • High school diploma or GED. Must be at least 18 years of age and able to pass a criminal background check under agency guidelines and state regulations.
  • Must be flexible to various individualized strategies towards supporting individuals with various needs.
  • Must possess a valid Nevada State Driver’s License. Vehicle liability insurance is required and clean driving record.
  • Must be comfortable in performing work duties independently with minimal supervision.
  • Able to demonstrate ongoing patience in a variety of challenging settings.
  • This position is contingent upon the successful completion of a background check (reference: NRS 449.174 and Medicaid Services Manual 2100).

RESPONSIBILITIES:

  • Coordinate and provide care that is safe, timely, effective, efficient, and individual-centered
  • Handle case assignments, draft service plans, review case progress.
  • Help individuals served achieve goals
  • Facilitate multiple care aspects
  • Help individuals served make informed decisions by acting as their advocate.
  • Develop working relations throughout the entire case management process
  • Record case information, complete all necessary forms
  • Must be able to work comfortably in a variety of employment settings.
  • Must be able to work under minimal supervision and as a member of a team.
  • Must possess good oral and written communication skills.
  • Must be able to work with a variety of individuals served, including those with disabilities.
  • Must have a flexible availability.
  • This position is responsible for participating as a team member providing services to adults experiencing intellectual, developmental and physical disabilities or severe emotional disturbances.  Work may be done in community or in the person’s home.
  • Services may include teaching skills, providing guidance and structure, prompting on personal care, household chores, Job site support and training, transportation, and providing respite support. Individuals served include adults experiencing a level of a disability that may cause them to be at risk in in the community without some level of support.  Work is performed within the context of the agency’s mission and core values.
  • Accesses resources necessary to implement the service plan in a manner sensitive to the individual, family and community. Makes adaptations according to specific developmental, mental health and medical needs.
  • Accurately completes daily service notes documenting services and progress toward achieving the individual’s plan goals using a smart device and online data entry.
  • Complete required paid training within scheduled time frame.
  • Provides pre and post-employment services to assist individuals served in gaining and/or retaining employment and initiating a career path leading to self-sufficiency.
  • Works with individuals served to evaluate individuals served  past assessments and update as necessary. Provides supports necessary to succeed on the job. Provides pre-employment services to ensure individuals served remain employed.
  • Provides/facilitates workshops in job keeping and life management skills.
  • Provides one-on-one support to find and maintain employment. Provides one-on-one support while on the job site to ensure job duties are fulfilled accurately and to the best of the supported individual’s abilities.
